We offer a dementia diagnostic service – specialist assessment, diagnosis and treatment for people with memory problems. Referrals to our team usually come following an individual’s consultation with a GP. This ensures that a physical reason for memory problems is ruled out and that some basic health screening has already been completed.
Adults who have ongoing memory problems with no physical health cause.
Initial assessment, Diagnostic appointment if appropriate, Post diagnostic follow up if appropriate prior to discharge to GP.
We don’t offer continued support or ongoing follow up after diagnosis.

We adapt our service to the individual’s needs making it inclusive to all, including home visits for people unable to attend clinics.
2-6 weeks for an initial assessment, 4-6 months for a diagnostic appointment; this is dependent on CT and MRI scans being completed and reported on via Mid Yorks hospitals and therefore out of our control.
Professional referral
Initial assessment – holistic assessment that gathers detailed information including background history, work, family, health, significant events, risks, strengths and current problems that have led to the referral. The assessment will determine if further memory investigations and a diagnostic appointment is required. Further tests may include a CT, MRI, or other scan dependent on age and presentation. Neuropsychology testing may also be considered. All this information will enable an informed decision-making process, diagnosis, treatment or signposting to other relevant services.
