Wakefield Intensive Home Based for Older People

We are a team of mental health professionals who work together to offer help and support for people aged over 65 years old who are experiencing a crisis of their mental health or require support with treatment in their own home.

Opening times / out of hours opening

  • Monday - Sunday : 8:00 am - 8:00 pm

Age range / who can access the service

Over 65 – Referrals are taken via OPS SPA  or internally froml Wakefield Mental Health services. The criteria is that intensive support  is required  to manage  a crisis or to prevent further relapse of symptoms

Key support offer

We offer short term intensive support and treatment between the hours of 8 am and 8 pm 7 days a week.   We will initially assess the service user at home and formulate a plan to support and treat significant mental health issues. This may be in the form of commencing / reviewing medication. Offering therapeutic support and signposting to other groups and services which may also support their recover. We can visit several times a day dependent on need. We attempt to support people as an alternative to hospital.

Social care support  ie personal care, physical health care.

Accessibility information for service

Service users are seen in their own home, if this is not possible arrangements can be made to meet with them at an alternative venue. We are able to access translators if required.

Waiting times / triage timescales

Service users are assessed within 4 hours or 24 hours dependent on level of risk / need.

Accessment process

Service users are assessed by a qualified member of staff in their own homes. The assessment will then determine   if support is appropriate to meet the need and  a plan will be formulated with the service user and cares to provide treatment in their own home.
