Wakefield Community Rehabilitation Team

We are a recovery and independence focused service that supports people to develop or rediscover life skills they may have lost due to their mental health. We support people to stay out of hospital by building confidence and personal resilience so people can manage the demands of their life alongside their mental health, independently.

  • Monday - Friday : 9:00 am - 5:00 pm

18 to 65

General support. Transporting people in our cars. Medication.

Accessibility information for service

All efforts will be made to make Community Rehabilitation Team accessible for all service users, through adaptation and adjustment.

Usually within two weeks of referral

Referral through allocated Care Coordinator or Lead Healthcare Professional

Accessment process

Standard assessment process lasts about 2 weeks and involves regular face to face visits from your allocated Occupational Therapist. The frequency of contact within this two weeks can be agreed with you, but generally we would see people daily or every other day during this time.

Upon discharge from Community Rehabilitation Team recommendations for other services and ongoing support will be made to your Care Coordinator to explore with you.
