Wakefield Intensive Home Based for Older People

The Intensive Home Based Team supports people aged 65 and over who are experiencing a mental health crisis. The team provides short-term support and treatment in your own home to help you recover and, where possible, avoid a stay in hospital.

Opening times

  • Monday - Sunday : 8:00 am - 8:00 pm

Age range

  • People aged 65 and over.
  • People experiencing a mental health crisis who need urgent support at home.

  • Mental health assessments at home.
  • Short-term support during a mental health crisis.
  • Medication reviews and support with treatment.
  • Emotional support and recovery planning.
  • Visits at home, including more than once a day if needed.
  • Help to access other local services and community support.

  • Personal care.

  • Long-term social care support.

Accessibility information for service

The team usually visits people at home. If this isn’t possible, another suitable place can be arranged. Interpreter services are also available if needed.

You will usually be assessed within 4 to 24 hours, depending on how urgent your needs are.

Accessment process

Service users are assessed by a qualified member of staff in their own homes. The assessment will then determine   if support is appropriate to meet the need and  a plan will be formulated with the service user and cares to provide treatment in their own home.

Key partners

The team works closely with GPs, Adult Social Care, Community Mental Health Teams, Admiral Nurses, Alzheimer’s Society and local community organisations.
