The Core Pathway

The Core Teams support adults with moderate to severe mental health needs living in the community. You’ll have a named healthcare professional who will work with you to develop a personalised care plan and support your recovery.

Opening times

  • Monday - Friday : 8:30 am - 5:00 pm

Age range

  • Adults aged 18–65 with moderate to severe mental health needs.

Key support offer

  • Mental health assessment and treatment.
  • Medication support and reviews.
  • Psychological therapies.
  • Recovery-focused support.
  • Psychiatrist appointments where needed.
  • Support to move back to primary care when appropriate.

What we don't offer

  • Crisis support.
  • Support for temporary life difficulties.
  • Routine medical follow-up for stable conditions.
  • Anger management or relationship counselling where specialist mental health support isn’t required.

Accessibility information for service

Support is available for people with language, hearing, sight or mobility needs, with text and email communication where appropriate.

Waiting times & timescales

Waiting times depend on the support you need. Initial appointments may take up to 8 weeks, with longer waits for psychological therapies.

Accessment process

After we have accepted a referral into the core team, we work with the individual to understand their mental health needs, their goals for recovery and to look at how the service can best support them. We will then develop a care plan which will include the identified needs and what their personal goals for their mental health are, working with the individual in beginning their recovery, reaching set goals, and to move towards discharge from mental health services.

Key partners

The team works with GPs, Turning Point, Recovery College, Wakefield Council, SPA and other specialist mental health services.
