Primary Care Mental Health Practitioner Team

The Primary Care Mental Health Practitioner Team supports adults experiencing common mental health difficulties that do not require specialist secondary mental health services. The team works with you to understand your needs and help you access the right support.

Appointments can take place at home, in a clinic, at your GP surgery or by telephone where appropriate.

  • Appointments: Monday - Friday : 9:00 am - 5:00 pm
  • Telephone: Monday - Friday : 12:00 am - 12:00 am
Team phone is available 8/4 and does not accept text messages.

Age range

• Adults aged 18 and over.
• People experiencing common mental health difficulties who have been referred by a healthcare professional.

You will usually be offered an assessment within 2–4 weeks of your referral.

Professional referral only, usually from a GP or VCSE organisation.

Accessment process

During your appointment you and a member of our team will have a chat. This helps paint a picture of you as a whole person and your individual needs. You will have the time in this appointment to explain how you are truly feeling. This will help both you and the team create the most suitable support plan. As part of the chat, you’ll complete some questionnaires to help build this picture.

Here are some examples of things we’ll discuss together

  • Any current concerns
  • Any anxiety or low mood you may be feeling
  • Any physical health issues you have
  • Any medications you take

We’ll find out more about your social situation – your finances, housing, family and friends’ network.› We’ll ask about your personal mental health history.› What’s worked or hasn’t worked for you in the past.› Your goals and what you want to achieve.› The things you enjoy doing and the opportunities in your local community.

Key partners

The team works closely with GPs, the Single Point of Access (SPA), Turning Point Talking Therapies, Social Care and Connecting Care to make sure you receive the most appropriate support.
