Admiral Nurses Wakefield

Admiral Nurses are specialist dementia nurses who support family members and unpaid carers of people living with dementia. They provide expert advice, emotional support and practical guidance to help carers manage the challenges of dementia.

• Adults aged 18 and over.
• Family members and unpaid carers of someone with a diagnosis of dementia.

• Crisis or emergency support.

Accessment process

After we receive referrals we might need to gather more information, either from professionals or from the carer to understand more about the support needed and who might be able to help. If Admiral Nurse support is needed we will offer a telephone, clinic or home visit appointment with one of our admiral nurses to find out more about your situation and the sort of help you need, appointments usually last about an hour.

We will decide together at this appointment what happens next. People can step in and out of the service when needed, so you might see us for a few months, until things feel more manageable, and then not have any appointments arranged. If things change and you need to talk to us again, you can contact us at any point.
