The Community Rehabilitation Team helps people build confidence and regain everyday living skills that may have been affected by their mental health. The aim is to support people to live as independently as possible and reduce the need for hospital care.
• Adults aged 18–65 who are receiving support from secondary mental health services.
• General support services.
• Transport.
• Medication management.

The team will make reasonable adjustments to help people access the service.
Most people are seen within two weeks of referral.
Professional referral only through your Care Coordinator or Lead Healthcare Professional.
Standard assessment process lasts about 2 weeks and involves regular face to face visits from your allocated Occupational Therapist. The frequency of contact within this two weeks can be agreed with you, but generally we would see people daily or every other day during this time.
The team works alongside Core Teams, Enhanced Teams, Insight Teams, Turning Point and the Recovery College.
