The Intensive Home Based Team supports people aged 65 and over who are experiencing a mental health crisis. The team provides short-term support and treatment in your own home to help you recover and, where possible, avoid a stay in hospital.

The team usually visits people at home. If this isn’t possible, another suitable place can be arranged. Interpreter services are also available if needed.
You will usually be assessed within 4 to 24 hours, depending on how urgent your needs are.

Service users are assessed by a qualified member of staff in their own homes. The assessment will then determine if support is appropriate to meet the need and a plan will be formulated with the service user and cares to provide treatment in their own home.
The team works closely with GPs, Adult Social Care, Community Mental Health Teams, Admiral Nurses, Alzheimer’s Society and local community organisations.
The team will help you access other services, such as your GP, Community Mental Health Teams, Adult Social Care or other local support.
